Our spacious 2-bedroom apartments are stunning with views of our beautiful landscape that is complete with a pond and waterfall. We accept Private pay, Medicaid, Long-term care insurance, and VA benefits. We have teamed up with a primary care provider and behavioral health specialist to promote a unique care model called Behavioral Health Integration (BHI). Our goal is to offer residents with dementia a comprehensive care plan that is integrated between a collaborative team of experts that each play a role in their well-being and success. How It Works: The collaborative team creates a comprehensive care plan custom tailored to each resident’s needs. Caregivers at White Pine, Comforts of Home and Gracewood utilize the care plan as a day to day guide on how to ensure each resident’s unique needs and wants are met. Consistent communication amongst the team serves as a tool to ensure seamless delivery of care occurs and outcomes are reached.
